Submitting a claim for a disability


Filing a disability claim is typically a three-step process.  The claimant needs to obtain a claim form and have the employer complete the applicable section.  The claimant completes his/her information, and then needs to have the doctor complete the Physician's Statement.  When all sections are completed, mail to the address on the form.

If you have become disabled and were covered by Disability Insurance, claim forms are usually available from the carrier's website.  We can help you access the forms most often used by Ochs, Inc. groups.
